In line with the Quebec government鈥檚 directive to reduce the strain on an already taxed healthcare system, students are no longer required to submit a medical note for incidental illnesses lasting fewer than five days.
This applies to all assessments鈥攊ncluding midterms, final exams, lab exams鈥攁s well as class absences.
Please note supporting documents are required for absences as a result of illness where the absence is for 5 or more day and听is still required for non-medical requests. Valid documentation is to be provided to the听Course Coordinator within FIVE working days of the original exam date.
This form should be used only make a request to defer听the final LAB exam for:

Fall semester: ANAT 214,听ANAT 261 and ANAT 315听

Winter semester: ANAT 314, ANAT 316 and ANAT 514
If you need to make a request to defer the听final WRITTEN exam, you must complete the request form in Minerva and follow the policies on the Exams website.

LAB exam requests will be reviewed听by the Course Coordinator
WRITTEN exam requests will be听reviewed听by Service Point (for students in the Faculty of Science) or your Student Affairs Office (for students in the Faculty of Education and School of Physical and Occupational Therapy)
